Avenues Mental Health Counseling

Payment Responsibility

Payment for services is due at the time of your appointment unless other arrangements have been made in advance. Clients are responsible for all fees associated with services received.

Accepted forms of payment may include credit card, debit card, Health Savings Account (HSA), or Flexible Spending Account (FSA) cards.

Insurance

If you are using insurance benefits, you are responsible for understanding your coverage, including:

  • Copayments

  • Coinsurance

  • Deductibles

  • Authorization requirements

  • Coverage limitations

Submission of claims does not guarantee payment by your insurance provider. Any balance not covered by insurance remains the client’s responsibility.

Missed Appointments and Late Cancellations

Appointments canceled with less than 24 hours’ notice or missed appointments may be subject to the full session fee.

Insurance companies do not cover missed appointment fees.

Outstanding Balances

Clients are expected to maintain a current account balance. Services may be paused if balances remain unpaid after reasonable notice.

Returned Payments

Returned or declined payments may result in additional processing fees and may require future payments to be made using an alternative method.
